What is a nursing home?

A nursing home is a long term care and/or a short term rehabilitative facility for those with significant health care issues and who require medical care.

When is a nursing home needed?
Your doctor or health care professional will speak to you, your family or your appointed representative as soon as the need for a nursing home is identified. It is usually a good time to research your options and alternatives. You may also speak to our Admissions Department at x148 or our Department of Community Relations at x224 to learn more about the UHGC campus of services and care options.

Are beds available?
Beds are often available. Bed selection is determined by the needs of the patient after review of the referral material. To inquire please call the Admissions Department at 632-2804 x148 or email knodiff@uhgc.org.

Can I take a tour?
Yes. We welcome you and/or your family or friends to tour. Tours can be scheduled seven days a week. Please call the admissions department for tour arrangements.

How are you accredited?
UHGC is accredited by the Joint Commission (JCAHO). This seal of approval indicates high performance standards. You can learn more about JCAHO by calling 630-792-5636 or visiting their website at www.jointcommission.org.

How is UHGC licensed?
UHGC is licensed by the New York State Department of Health. We are surveyed annually and must meet strict standards of care.

What is the admission process?
Referral material is reviewed by the admissions committee. If the patient meets admission criteria and an appropriate bed is available, admissions arrangements are made.

What methods of payment do you accept?
We accept Medicare, Medicaid, managed care (HMOs) and private pay.

Do you have anyone to help with the insurance and the Medicaid process?
Yes. We have a reimbursement specialist available to assist you.

Do you offer short term rehabilitation?
Yes. The UHGC rehabilitation department is managed and staffed by Burke Rehabilitation Hospital, nationally renowned rehabilitation specialists. We offer physical therapy, occupational therapy, speech therapy and more. In consultation with your physician, our health care team will work to help you return home as quickly as possible. Do you admit Alzheimer’s patients and people with dementias?
Yes. People with memory impairment are admitted to the appropriate unit. We also offer the Phoenix Neighborhood at Willow Towers Assisted Living for those with memory challenges.

May residents worship at UHGC?
UHGC is a non-sectarian facility and we offer a variety of religious services.

Are personal physicians allowed?
Yes, your personal physician is welcome at UHGC.

How are families involved in the treatment plan?
Families are invited to scheduled meetings and participate in the formulation of the treatment plan.

Are other medical services available at UHGC?
Yes, Medical services such as ophthalmology, podiatry and dentistry are available.

How is quality maintained at UHGC?
The UHGC staff are trained to abide by our mission of service excellence. A satisfaction survey is completed by all residents and employees annually. We are surveyed by JCAHO and the New York State Department of Health annually. Our Performance Improvement Committee meets regularly to continually work to maintain our quality care.

What do I do if I have a question about quality care?
Please call our Director of Regulatory Affairs. Unresolved issues of concern are brought to the Performance Improvement Committee for review and resolution. If you have any further questions about care concerns you can contact JCAHO at 630-792-5636.

How will I get my medication?
UHGC has a pharmacy on site. The pharmacist will dispense the medications ordered by your physician.

Do you offer assisted living?
Yes, Willow Towers is our assisted living community on our campus. It is a supportive living environment for those that want apartment living with amenities and assistance available for personal care needs. Residents receive three meals per day, recreational activities, scheduled transportation to doctor visits, daily housekeeping and more. A nurse is available on site.
