Long Term Home Care

Our not-for-profit, JCAHO accredited, Long Term Home Health Care Program also known as The Lombardi Program, has provided individuals with an alternative to nursing home placement since 1984.

Our program offers options for those requiring skilled services by enabling them to receive care in their own home under the supervision of their physician and our coordinated health care team. Our staff is on call 24 hours a day to meet your needs.

Services include:

• Registered nurses and case managers
• Licensed social workers
• Physical, occupational and speech therapists
• Nutritionists
• Home health aides
• Personal care workers
• Medical supplies and equipment
• Medication management and laboratory work
• Home modifications to meet a clients' changing needs

Who is eligible?

Chronically ill or disabled Westchester County residents who are under a physician’s care and can safely remain at home with part-time services. The program is open to Medicaid, Medicare and private pay individuals.
